and disparities. Variables influencing the risk and disparity experienced by members of the vulnerable group will be
identified, and correlation with national directives to address population health issues will be determined. A concept map
will be created to portray the relationships between these elements. Implications for advanced nursing practice and
opportunities to advocate for the health of the vulnerable population will be presented.

Identify one vulnerable population within a selected community. Explore current literature and related data to better
understand the variables that place this population at risk for health concerns and health disparities. Analyze national
health directives (such as Healthy People or another national initiative) to determine correlation with
needs/risks/disparities of the vulnerable population.
Part 1: Concept Map
1. Identify the following elements to include in the concept map.
a. Vulnerable population
b. Variables (risk factors) that place the identified population at risk (minimum of three)
c. Minimum of one health risk or disparity for which the population is at risk
2. Create a concept map that portrays each element and the relationships between the vulnerable population, variables
placing the population at risk, and the identified health risk or disparity for which the population is susceptible. The
